Wo hast du diese Erkenntnis her?

Although considered an off-label use for treatment, Viagra has also helped with premature ejaculation particular those who suffer from intermittent impotence as well.

Of course I now rely on Viagra which can result in an occasional lack of ejaclation.

premature ejaculation (PE) - Premature ejaculation is the inability to maintain an erection long enough for mutual satisfaction. Premature ejaculation is divided into primary and secondary forms:
primary premature ejaculation - Primary premature ejaculation is a learned behavior that begins when a male first become sexually active. Like any learned behaviors, it can be unlearned. This form of primary PE is psychogenic (as opposed to organic or physical) impotence. (Congenital venous leak is a subset of primary PE and is caused by a congenital venous leak in which the venous drainage system in the penis does not shut down properly.)
secondary premature ejaculation - Secondary premature ejaculation occurs when, after years of normal ejaculation, the duration of intercourse grows progressively shorter. Secondary PE is due to physical causes, usually involving the penile arteries, veins, or both.
Martin Irvine: OK, thank you. I think we can now announce the results of the poll. OK, so 34% have recommended self-help or psychological approaches for their patients. Five percent have recommended topical treatment, 13% PDE-5 inhibitors, and 43% SSRIs, with 3% recommending other approaches. Would you care to comment, Dr. McCullough?
Andrew McCullough: Well, that's very interesting. So people have already kind of noticed that the SSRIs do have a beneficial affect on ejaculatory latency, which is, as Jamie [Dr. Barada] said, one of the side effects of the SSRIs. I think it's interesting that 13% have also used sildenafil as a treatment, and we'll go over that shortly. But it's interesting that one of the...I did the original...participated in the pivotal trials for sildenafil, and one of the patients said, "Gee doctor, not only did you improve my erection, but you improved my premature ejaculation." So, I think that it's an interesting side effect of this medication.
What about PE and sildenafil? Well, what does sildenafil do? It allows the erection to occur with less stimulation. So theoretically, the man who is very excitable will have an erection with less thrusting, with less stimulation, and in fact, because of that, he can do his calculus exercises, or think about the NBA finals, and not have an ejaculation.
So, that is a possibility. It also decreases the refractory period for erections. So just like the young man who goes and masturbates before he's sexually active, the older man will now be able to do that as well, and recover his erection and then have a longer refractory period. And finally, there's a knockout mouse model for the gene that has the codes for endothelial nitric oxide synthase. These mice develop a condition similar to premature ejaculation.
So there is a theoretical basis for the use of sildenafil, but this has been looked at, and to my knowledge, there have really been no studies to show that in terms of stopwatch times in a controlled way, there has been any benefit from the use of sildenafil. It doesn't mean that doesn't improve the perception of control, but it's more difficult to show in terms of stopwatches.

CONCLUSIONS: Paroxetine combined with sildenafil appears to provide significantly better results in terms of ejaculatory latency time and intercourse satisfaction versus paroxetine alone in potent patients with premature ejaculation.

Viagra, Paxil Help Premature Ejaculation
Once again, Viagra comes to the rescue. A new study shows that Viagra in combination with Paxil helps premature ejaculators who have not improved with standard treatment.
One possibility is that Viagra acts on the part of the brain that controls ejaculation. This function involves the chemical nitric oxide. Too much nitric oxide inhibits ejaculation. Viagra increases the amount of nitric oxide in the body, which is how it helps men get erections. Juza says there is some scientific evidence that Viagra increases the amount of nitric oxide in the brain, although that is not certain.
"Another possibility could be a decrease of sensitivity in the [head of the penis]," Juza says.
Arnold Melman, MD, head of the urology department at the Montefiore Medical Center in New York City, says he doubts that has anything to do with it. "What evidence is there?" he says. "Do you think when you have a hard erection you're less sensitive?"
Viagra may have had a psychological effect, too: With a firmer erection, they had more confidence, so they had more control. Melman says he thinks that is a more likely explanation. "I'm sure that's true," he says. "Much of the reason for premature ejaculation is psychological, not physical."
Juza says more research is needed to learn about using Viagra as a treatment for premature ejaculation.

Clinical efficacy of Viagra with behavior therapy against premature ejaculation
CONCLUSION: During this clinical trial, Viagra combined with behavior therapy prolonged IELT, which suggests that Viagra may be helpful for the treatment of premature ejaculation.

Dr. Laurence Levine, a urologist who sees more than 7,000 men in his Rush University Medical Center practice, said Viagra and its competitors will delay orgasm. That helps men with premature-ejaculation problems.
Naja...
